What Is The Purpose Of An Advance Health Care Directive?

The purpose of an advance health care directive is to protect your choices when you cannot communicate. It can also reduce stress for your loved ones by giving them guidance to follow.

An advance directive often includes one document or a set of documents, depending on your state. In many cases, it covers both treatment preferences and who can speak for you.

1. Living will: States your treatment preferences if you are permanently unconscious or have a terminal condition. Many people use it to document pain relief, artificial nutrition, and hydration choices.
2. Medical power of attorney: Names an agent who can make medical decisions for you. If you want this option, use a medical power of attorney alongside your advance directive, if your state uses separate forms.
3. Do-not-resuscitate order: Tells providers not to attempt CPR if your heart or breathing stops. This is often a separate medical order and may require a clinician’s involvement, depending on your state.

Does An Advance Directive Guarantee Your Wishes?

Step 1 – Make Your Declaration

Identify yourself and confirm you are of legal age and able to make your own decisions. Many people complete advance directive forms before surgery, during long-term treatment, or as part of routine planning.

Step 2 – Choose A Health Care Agent

Pick someone you trust to make medical decisions for you if you cannot. You can also set limits, such as when your agent may consent to certain procedures.

If your state allows, you may also nominate a guardian. A guardian is a court-appointed role, and courts often consider your nomination.

After you sign, store the original in a safe place and share copies with your agent and your health care provider. Also keep a copy that is easy to access in an emergency.

Update your advance directive when life changes, such as a new diagnosis, a major relationship change, or a move to a new state. When you replace an old directive, share the updated version and clearly revoke older copies to prevent confusion.
