Name one or two areas honestly, with an example attached to each. A long list dilutes the message.

Attendance and Reliability

Record only what your systems actually track. Otherwise this section drifts into impression rather than fact.

Support Needed

Note training, tools, or access the employee needs. This turns criticism into a plan.

Goals for the Next Period

Set a small number of specific goals, each with a date attached.

Comments and Signatures

Leave room for the employee’s own comments, then both signatures and the date.

Written comments outlive the meeting, so assume somebody else reads them later.

Describe behavior and results rather than character. Missed three deadlines this quarter says something useful, while unmotivated does not.

Date the examples you use. A specific month makes a comment verifiable, and it stops old issues resurfacing indefinitely.

Quote figures where you have them. For example, a delivery rate carries more weight than a general sense of pace.

Separate performance from circumstances. For instance, a project that failed for reasons outside somebody’s control belongs in context.

Keep the language plain. Jargon reads as evasion, and an employee should not need help interpreting their own review.

Balance the record honestly. A page of praise followed by a poor rating confuses everybody who reads it.

Avoid comments touching health, family, or personal characteristics. Keep the review on the work itself.

Apply the same standard across a team. Inconsistent ratings between similar performers are difficult to defend afterward.
