A construction incident report is a written record of an unplanned event on a jobsite, from an injury or near miss through to equipment damage. It also captures the task underway, the conditions, the people present, and the response.

Jobsites change hourly, which makes this record harder to rebuild after the fact than most. So its value comes almost entirely from writing it while the scaffold, the weather, and the crew list still match what you saw.

The form describes events. Therefore, it does not apportion fault between the general contractor, a subcontractor, or a supplier.

File a construction incident report for any event that injured someone, could have injured someone, or damaged equipment or the structure. Near misses count as much as injuries here, because the same gap usually produces both.

Typical triggers on site include:

• Someone falls from a ladder, a scaffold, or an unguarded edge.
• Falling material or a swinging load strikes a worker below.
• A trench wall slumps, or shows early signs of movement.
• An excavator contacts a buried utility line.
• A power tool kicks back, or a guard fails mid-cut.
• Lifts, cranes, or hoists behave unexpectedly.
• Heat exhaustion symptoms appear during a hot shift.
• A subcontractor works outside the permit or the method statement.

Log the no-injury events too, such as a wrench dropped from the third floor. Also, those entries build the pattern that justifies a fix.

Tip: Keep near misses on the same form as injuries. Sites that park near misses in a separate notebook usually lose them, and near-miss data is the cheapest safety information a project ever gets.

A construction incident report needs more context than a general one, because who employed whom and what was underway both shape the outcome. So work through these fields before the shift ends.

Project and Location

Record the project name, the site address, the current phase, and the precise area: level, grid reference, zone, or unit number.

Date, Time, and Shift

Log the date, the clock time, and which shift was running. Note how far into the shift the event landed, since fatigue patterns show up here.

People and Their Employer

Name everyone involved, with their trade and their employer. On multi-employer sites this field matters more than any other, so never shorten it to a first name.

Task Underway

Describe exactly what the crew was doing at that moment. Also include the work method, the permit in force, and any deviation from the plan.

Equipment, Plant, and Materials

Identify the plant or tools involved by asset number or serial number. Add the last inspection date if your records carry it.

Site Conditions

Capture weather, ground condition, lighting, noise, access, and anything else that made the task harder that day.

Sequence of Events

Walk through what happened, in order, in plain language. Then cover the lead-up, the event, and the response, and mark unknowns as unknown.

Injuries and First Aid

Record injuries, treatment given on site, and whether anyone left for further care. Then note who administered first aid.

Damage to Plant or Structure

Describe damage to equipment, materials, temporary works, or the permanent structure, and note whether the area stayed usable.

Immediate Actions Taken

Log what happened straight away: work stopped, equipment isolated, the area barricaded, or the crew stood down.

Witnesses

Name witnesses with their employer and contact details, and summarize what each one saw.

Permits, Inspections, and Briefings

Reference the permit, the last inspection, and the most recent toolbox talk covering this task. Reviewers reach for these first, so keep the references exact.

Report up the chain the same day, so start with whoever runs safety on site. The person who saw the event and the person who must report it are often not the same on a multi-employer project, so name both.

A workable order looks like this:

1. Make the area safe, and isolate any equipment that played a part.
2. Tell the site supervisor or the safety lead straight away.
3. Complete the written record before the shift ends.
4. Pass copies to the general contractor and to the injured worker’s employer.
5. Confirm who owns any external reporting duty.

Serious injuries and structural failures often carry short external deadlines. Those rules depend on your trade and on where the site sits. So confirm what applies with the safety regulator covering your project, rather than assuming it matches the last job.

A record nobody acts on is just paperwork. So close every one with a named fix, a named owner, and a date.

• Immediate control — what stopped the hazard today, such as barricading an open edge.
• Root cause — the condition or decision that allowed it, not the person standing nearest.
• Corrective action — the change that prevents a repeat, like a redesigned lift plan.
• Owner and date — who does it, and by when.
• Verification — how somebody confirmed the fix held.

Then feed the finding into the next toolbox talk. Crews take a control seriously once they hear the event behind it.

A construction incident report is an internal safety record, so it does not satisfy an insurance claim or an official filing on its own. Instead, treat it as the source your other obligations draw from.

Requirements vary by state, by trade, and by the contract you signed. Therefore, check your state’s official website, read the reporting clause in your contract, and confirm expectations with your insurer before an event happens rather than after.

Also keep these in mind:

• Injury details are sensitive, so restrict the file to those who need it.
• Workers are generally protected when they raise safety concerns, and discouraging a report creates a risk of its own.
• Photographs and daily logs strengthen the record, so store them with it.

For a fatality, a structural collapse, or anything likely to end in litigation, involve your attorney and your insurer immediately.
