A patient incident report is a written record of an unexpected event involving a patient in a hospital, clinic, care home, or home-health visit. It captures what happened, who was there, and what staff did next.

Care teams use these records to review problems and improve safety. Therefore, the purpose is documentation, not blame.

Any staff member can file one. Nurses, aides, therapists, and technicians all write them, and so do managers reviewing an event afterward.

The report also stays strictly factual. For example, it notes that staff found a patient on the floor beside the bed, instead of guessing why the fall happened.

Use a patient incident report whenever something happens outside the normal course of care. Most teams file one during the same shift, while details are still clear.

Common triggers include:

• Falls: A patient slips, trips, or ends up on the floor.
• Medication events: A wrong dose, a missed dose, or an unexpected reaction.
• Treatment delays: A test, transfer, or procedure that did not happen on time.
• Equipment problems: A pump, lift, monitor, or bed rail that failed during use.
• Missing belongings: Hearing aids, dentures, glasses, or valuables nobody can locate.
• Wandering: A patient leaves a unit without staff knowing.
• Aggression or threats: Conflict involving a patient, a visitor, or a staff member.
• Exposure events: A needlestick, a spill, or contact with a hazardous substance.

Also file a report for close calls. A wrong dose caught before it reached the patient still points to a gap worth fixing, and near misses often expose the same weak step as an event that caused harm.

A complete patient incident report answers who, what, when, where, and what followed. Keep each answer short and specific.

Patient and Reporter Details

Record the patient identifier your facility uses, along with the unit or room. Then add the name and role of the person filing the report.

Date, Time, and Location

Note when the event happened and when someone discovered it. Those two times often differ, and the gap matters during review.

What Happened

Describe the event in plain language. Stick to what someone saw, heard, or measured.

Condition and Response

Record the patient’s condition right after the event, plus any assessment, first aid, or treatment given. Also note which clinician you contacted, and when.

Witnesses

List anyone who saw the event, including staff, visitors, or other patients. Then add contact details in case questions come up later.

Equipment and Supplies

If a device played a part, record its type and identification number. Also note whether it went out of service.

Notifications

Write down who heard about the event: a supervisor, the attending clinician, the patient, or a family member. In addition, log the time of each notification.

Follow-Up Actions

Close with steps already taken or planned, such as a safety check, a repair request, or extra monitoring.

Work through the report in order, and finish it before your shift ends.

1. Care for the patient first. Address injuries, call for help, and make the area safe before you write anything.
2. Collect the basics. Note times, locations, room numbers, and everyone present.
3. Write the sequence. Describe the event step by step, from what led up to it through how it ended.
4. Quote, don’t summarize. If a patient or visitor described what happened, record their words in quotation marks.
5. Add the response. Include the assessment, treatment, and notifications, along with the time of each.
6. Review before you submit. Check names, doses, and times, then fix anything unclear.

Because these records feed safety reviews, accuracy matters more than polish. So a short, precise account beats a long one written from memory a day later.

An incident report is not part of the patient’s chart. It documents an event for internal review, while the chart documents the care a patient received.

The two files also reach different readers. Charts follow the patient through treatment. Meanwhile, these reports go to the people who study safety trends across a unit.

Clinical facts still belong in the chart. Record the assessment, treatment, and patient response there, exactly as you would for any other care.

Timing helps as well. Chart the care while it happens, then complete the separate report once the patient is stable and the immediate work is done.

Tip: Many facilities ask staff to keep any mention of the report out of the clinical record. So check your own policy before you document either one, and ask your supervisor if the answer is unclear.

Records about patients hold sensitive details, so handle them carefully from the first line.

Limit Who Can See It

Share the report only with people who need it, such as a supervisor or the risk and quality team. Also follow your facility’s privacy policies for storing and sending it.

Write Facts, Not Opinions

Describe what happened without assigning blame or guessing at causes. If something is unknown, say so plainly.

File It Quickly

Complete a patient incident report while details are fresh. Memory fades within hours, and a late account is harder to confirm.

Know Who Reads It

Most facilities route these reports to a risk, quality, or safety committee. That group looks for patterns rather than fault, so write for a reader who wants to prevent the next event.

Ask Before You Assume

Reporting duties differ by employer, by care setting, and by state. Therefore, confirm what applies to you with your supervisor, your compliance team, or your state’s official website.

A serious injury or a threatened claim is worth reviewing with your facility’s legal or risk team.
