A PA school letter of recommendation is a written evaluation supporting an application to a physician assistant programme. It sets out how you know the applicant and what you have seen of their clinical aptitude, judgement, and character.
PA programmes are compressed and demanding, and most expect applicants to arrive with substantial hands-on patient care experience already behind them. Committees therefore look hard for evidence from people who have watched the applicant with patients.
Programmes usually ask for two or three letters and often specify the mix. Requirements differ, so check what each programme accepts before agreeing to write.
The letter also arrives alongside a documented record of the applicant’s clinical hours. Your job is not to restate those numbers but to describe what the applicant was actually like during them, which is the part no form can capture.
The identity of the writer matters more here than in most applications. Strong choices include:
A letter from a practising PA carries particular weight, because that writer can speak directly to whether the applicant suits the role. If the applicant has worked alongside one, that is usually the letter worth pursuing first.
A PA school letter of recommendation should run one to two pages, and it should lead with what you personally observed.
Give your name, credentials, role, organization, and contact information.
Describe where you worked with the applicant, over what period, and how closely.
This is the section committees read first. Describe how the applicant behaves with patients, particularly frightened, confused, or difficult ones.
Comment on whether they recognize the limits of their knowledge and escalate appropriately.
Note how they handled scientific material or technical training, since PA programmes move quickly and expect students to keep pace from the first week.
PA practice is collaborative by design. Describe how the applicant works within a care team, including with staff more senior and more junior than they were.
Address attendance, preparation, documentation, and conduct.
Close with a plain recommendation and a contact route you monitor. Say whether you would want them in your own clinic once qualified.
Most of what makes a PA letter persuasive comes from describing the applicant’s hands-on work accurately.
Be specific about the setting and the level of responsibility. A committee reading that someone worked in a clinic learns little; reading that they roomed patients, took histories, and handled triage calls for two years learns a great deal.
Describe the difficult moments. How an applicant behaved when a patient was distressed, when a shift ran short-staffed, or when they were corrected in front of others says more than a summary of their duties.
Avoid inflating scope. Suggesting an applicant performed tasks beyond their actual role damages credibility and can create problems for the applicant later, particularly if a committee asks them about it at interview.
Keep patient details out. Describe the situation and the applicant’s behaviour in terms general enough that no individual could be identified, which is both the right thing to do and perfectly sufficient for a committee.
Tip: Ask the applicant to remind you of specific cases you both remember, with identifying details removed. A described encounter, kept general, is the most useful thing you can give a committee.

One thing consistently weakens otherwise strong letters: a recommender who writes as though the applicant were applying to medical school.
PA training and practice are their own thing. Describing the applicant as a future doctor, or framing the PA route as a step toward something else, suggests the writer does not understand the profession and can undercut the applicant’s stated motivation.
Write about collaborative practice, breadth, and adaptability. PAs work across specialties and within teams, so evidence that the applicant thrives in that structure is directly relevant.
If you are unsure about the role, ask the applicant to explain why they chose it. A short conversation prevents the most common misstep and usually surfaces material worth including.
It also helps to name what the applicant will bring to a team rather than only what they have achieved. PA programmes are selecting future colleagues for other clinicians, and evidence that someone makes a service run better is directly on point.
