A residency letter of recommendation is a written evaluation of a medical student or graduate applying to postgraduate training. It comes from someone who supervised them clinically and describes what they observed on the wards or in clinic.
Programme directors read these to answer a practical question: what will this person be like to work with, and to supervise, from day one. That makes direct clinical observation the currency, and letters from writers who never worked with the applicant carry little weight.

Some specialties have moved away from free-form letters toward structured evaluation formats, and knowing which applies saves considerable rework.
Where a standardized format is used, you will typically be asked to rate the applicant against defined competencies and answer set questions, often through a specific portal with its own deadline. The format is designed to make applicants comparable across writers, so answering thinly stands out.
Structured formats also reward honesty. Because ratings are compared across many writers, marking every applicant at the top flattens the signal and tends to be discounted, while ratings that vary sensibly and match the written comments carry real weight.
Where a narrative letter is expected, specialty fit should still be explicit. A programme director wants to know that you have thought about whether this applicant suits this specialty, not merely that you liked them.
