Free Utah Notary Acknowledgment Template

A Utah notary acknowledgment is a notarial act defined in Utah Code § 46-1-2. In it, the notary certifies that you admitted, in their presence, to voluntarily signing a document for its stated purpose. The notary must know you personally or have satisfactory evidence of your identity. The Notaries Public Reform Act, Title 46, Chapter 1, governs it.

Purple-Ink Stamp

According to the Lieutenant Governor’s office, a rectangular notary stamp uses purple ink within a border no larger than 1 inch by 2.5 inches. It includes the notary’s name exactly as commissioned, “notary public,” and “state of Utah.” Plus, it shows the commission number, “my commission expires on” with the date, and a facsimile of the Great Seal of the state.

Seal Plus Certificate

A Utah notary may not use their seal independent of a notarial certificate. In other words, every notarization needs the seal, notarial language, and the notary’s signature together.

Fees

A notary may charge up to $10 per signature for an acknowledgment, or up to $25 for a remote notarization (§ 46-1-12). The notary may also add a travel fee, capped at the federal mileage rate, if you agree to it. Since fees can change, confirm them first.

The notary must sign the certificate exactly and only with the name on their commission (§ 46-1-16).

No. Utah’s definition of satisfactory evidence of identity specifically excludes a driving privilege card under Subsection 53-3-207(10). It also excludes any other document that isn’t considered valid for identification.

That matters if a driving privilege card is the card you carry every day. In that case, plan ahead and bring a different government ID, such as a passport.

The Lieutenant Governor’s handbook also reminds notaries that an acknowledgment still requires personal appearance and proof of identity. A phone call, fax, or email doesn’t count.

Once the certificate is ready, the visit is short.

1. Attach or confirm the § 46-1-6.5 acknowledgment wording.
2. Fill in the county, date, and your name.
3. Bring valid ID other than a driving privilege card.
4. Admit to the notary, in person, that you signed the document voluntarily.
5. The notary signs exactly as commissioned.
6. The notary applies the purple-ink stamp next to the certificate.

Utah Code § 46-1-6.5(3) sets out the acknowledgment form. After “State of Utah” and a county line, the key sentence follows: “On this (date) day of (month), in the year (year), before me (name of notary public), a notary public, personally appeared (name of document signer), proved on the basis of satisfactory evidence to be the person(s) whose name(s) (is/are) subscribed to in this document, and acknowledged (he/she/they) executed the same.”

A Utah notary acknowledgment in that form then includes:

• The signature of the notary
