Free Military Power of Attorney Form

A military power of attorney (POA) is a legal document that lets a service member (the principal) appoint an agent to handle specific matters for them. Once signed correctly, the agent can act on the principal’s behalf for the tasks listed in the document.

Many service members use a military POA before deployment, training, or reassignment, especially when time zones, limited connectivity, or mission demands make it hard to manage life back home.

Federal law recognizes military powers of attorney across the US, which helps reduce state-to-state paperwork issues (10 USC § 1044b)

A military POA can be broad or very limited. You can use it to cover one task, a category of tasks, or multiple ongoing responsibilities.

Durable Power of Attorney

Springing Power of Attorney

A springing POA only becomes effective after a specific event happens. For example, you might want it to activate when you deploy or if a doctor confirms incapacity.

A strong military POA is clear about who is involved, what your agent can do, and when their authority begins and ends.

Step 1 – Add the Principal and Agent

Include your full legal name and contact information, then add your agent’s full legal name and contact details. Also name a backup agent if you want extra coverage.

Step 2 – Choose the Powers You Want to Grant

Decide what your agent can do. Keep it as narrow as possible for the task, especially if you’re authorizing access to money, property, or contracts.

• Any limits on dollar amounts, accounts, or property

Step 3 – Set When It Starts and Ends

You can make the POA effective immediately, on a specific date, or after an event. You can also set an end date or end it after a task is completed.

Step 4 – Add Special Instructions

Use special instructions to reduce confusion. For example, you can require your agent to share receipts, notify you after signing, or limit actions to a specific organization.

Step 5 – Sign and Notarize
