Free Colorado Motor Vehicle Power of Attorney Template

A Colorado motor vehicle power of attorney appoints an agent to act on one vehicle’s title and registration matters. The Division of Motor Vehicles publishes Form DR 2175 for this, and the form is notarized, dated, and limited to the powers you tick.

Form DR 2175, headed Power of Attorney for Motor Vehicle Only, cites C.R.S. 15-14-701 and 15-14-705. The form explains its own scope, and it is blunt about what voids it.

Eight Powers, Chosen by Checkbox

You tick only the powers your agent needs, and may cross out those withheld. The list runs:

• Apply for and receive a new registration or new temporary registration
• Apply for and receive a certificate of title
• Apply for and receive a duplicate certificate of title
• Transfer ownership and acknowledge the odometer reading
• Record a lien
• Release a lien
• Apply for and receive a copy of a motor vehicle record
• Receive a Persons with Disability parking privileges placard

A Termination Date Is Required

Notably, the form says so in capitals. It terminates on a date you write in, so an open-ended grant is not an option here.

Original or Copy, Depending on the Power

Colorado splits this neatly. The original must be surrendered to transfer ownership and acknowledge the odometer reading, while certified copies are acceptable for all other powers. The agent may keep a copy for record keeping.

Also, the form repeats the point further down. A third party may act on a copy unless a transfer of ownership is occurring, and all transfers of ownership require the original.

One Signer Only for Odometer Disclosure

Meanwhile, there is a hard limit on mileage. Where the agent will complete the odometer disclosure as buyer only, or as seller only, this non-secured form may be used.

It cannot be used for both sides. Signing as buyer and seller, disclosing and acknowledging the odometer reading, requires the DR 2174 Secure Power of Attorney instead.

Identification and Warnings

The grantor gives a printed name as it appears on identification, plus the signature and date. Identification details follow: a Colorado driver’s license or ID or another type, with number, expiry, and date of birth.

Two warnings sit on the face of the form. First, any alteration or erasure may void the document. Second, by exercising the powers, the agent assumes the fiduciary and other legal responsibilities of an agent under Colorado law.

Yes, the form has a notary block. DR 2175 closes with wording that the document was subscribed and affirmed, or sworn to, before the notary. The grantor executed it in the notary’s presence, and presented the identification described above.

However, that last phrase matters. The notary records the identification you showed, so bring the license or ID whose number you wrote on the form.

At least one online guide claims Colorado does not require notarization for vehicle-only powers of attorney. The form itself contradicts that, and the notary block carries a signature, printed name, commission expiry, and seal.

Tip: Do not correct a mistake with a pen stroke or correction fluid. Any alteration or erasure may void your Colorado motor vehicle power of attorney, so start again on a fresh form.

As always, decide the powers before you reach the notary. Follow these steps:

1. Enter the grantor’s and agent’s full names as they appear on identification.
2. Tick only the powers the agent needs, and cross out any withheld.
3. Write the termination date, since the form requires one.
4. Add the VIN, year, make, and model, plus your identification details.
5. Sign in front of a notary, showing the identification you listed.
6. Give the agent the original for a transfer of ownership, or a certified copy otherwise.

Here too, the form’s own fields make the checklist.

• Grantor’s full name as it appears on identification
• Agent’s full name, or entity name
• The powers granted, ticked individually
• Termination date
• Vehicle identification number, year, make, and model
• Grantor’s identification type, number, expiry, and date of birth
• Printed name, signature, and date
• Completed notary block with seal

Document Genius’s guided form walks you through each field. So your Colorado motor vehicle power of attorney is ready to print and take to the notary.
